Court forms
If family mediation isn’t appropriate or has broken down, you or your former partner may need to apply to the court. In these cases, a judge can make decisions about important matters such as children, property, or finances. Below, you’ll find links to commonly used court forms available for download.
Form C100 – Application for a Child Arrangements Order
Use this form if you’re asking the court to decide on arrangements for a child (e.g., where the child lives, who they spend time with, or other parental responsibilities).
Form FM1 – Family Mediation Information and Assessment Form
This form shows that you've considered mediation before applying to court (or explains why mediation wasn't appropriate). It's usually required when submitting a C100.
Form D8 – Divorce or Dissolution Application
Use this form to start divorce or civil partnership dissolution proceedings.
Form Form A – Notice of [Financial] Application
Submit this form if you're asking the court to resolve financial matters after a separation or divorce (e.g., division of assets, maintenance).
Form E – Financial Statement
This detailed form is used to disclose your financial circumstances when financial claims are being made (typically required after a Form A is submitted).
Form C1 – Application for an Order under the Children Act 1989
Used for other types of applications involving children that aren’t covered by a C100, such as appointing a guardian.
Form EX160 – Application for Help with Court Fees
Use this if you’re on a low income or receiving benefits and want to apply for reduced or waived court fees.
